NOTE: The forms below are in Adobe Acrobat format. The Adobe Acrobat Reader is available as a free download.
- Complete the D.Min. Online Application, saving as you go, and submit via email. You will find instructions on the first page of the application.
- Submit other components of the application (listed here) via mail.
If you would rather complete the application for admission by hand, please print this version of our application. You may also contact the admission office by emailing bdadmis@samford.edu or by calling 1-877-575-0595 or 205-726-2066, and we will send you a hard copy of our application.
Attention Mac Users:
The online application document must be opened in Adobe Reader, not in Preview for Mac. Although you will be able to see it in Preview, you will not be able to sign it. If you don’t have Adobe Reader, you may download it for free at http://get.adobe.com/reader.
